Heart team approach for patients with unprotected left main coronary artery disease.

چکیده
atients with unprotected left main coronary artery disease (LMCA) have a worse prognosis than any other form of coronary artery disease (CAD). During the past decade, the efficacy and safety of percutaneous coronary intervention (PCI) vs. coronary artery bypass grafting (CABG) for LMCA have been major topics for both interventional cardiologists and cardiac surgeons. CABG has been considered as the standard treatment modality for these patients, but as stent design, procedural technique and adjunctive medical therapy have improved, PCI has emerged as an alternative treatment option. Accordingly, PCI with stenting for LMCA was upgraded to a class IIa recommendation for properly selected patients in the 2012 guidelines of the United States.1 However, selection of optimal treatment for these patients remains controversial because of different clinical decision pathways and complicated evaluation of coronary anatomy and morbidity at the time of intervention.

Acute and Long Term Outcomes of Coronary Intervention in Unprotected Left Main Lesions
Introduction : Significant left main coronary artery stenosis jeopardizes the entire myocardium of the left ventricle and has the worst prognosis of any form of coronary artery disease. Coronary-artery bypass grafting (CABG) has been considered as the standard therapeutic approach for such patients. There are limited data on the safety and effectiveness of percutaneous coronary intervention (PC...
متن کاملLeft Main Coronary Artery Disease: traditional risk factors in a study from northwest of Iran
Background & Aims: Significant left main coronary artery disease (LMCAD) is found in 3 to 6 percent of all patients who undergo coronary arteriography. LMCAD usually requires an emergent surgery that has a higher rate of mortality and complications. The risk factors of left main involvement in previous studies are controversial. The aim of this study was to determine the traditional risk factor...

OBJECTIVES To evaluate the long-term results in elderly patients undergoing percutaneous coronary intervention (PCI) with drug-eluting stents for unprotected left main coronary artery disease by transradial approach. METHODS This study took place in Qinhuangdao First Hospital, Hebei Medical University, Hebei, China between October 2006 and December 2009. Seventy-nine elderly patients with unp...
متن کاملTreatment of left main coronary artery stenosis with drug-eluting stent following heart transplantation.
Cardiac allograft vasculopathy is the leading cause of death after the first year of heart transplantation. The optimal treatment for unprotected left main coronary artery disease in orthotopic heart transplantation (OHT) patients is unknown. Two OHT patients with left main disease following heart transplantation underwent percutaneous coronary intervention (PCI). Technical success was achieved...
متن کاملBayesian methods affirm the use of percutaneous coronary intervention to improve survival in patients with unprotected left main coronary artery disease.
BACKGROUND Several randomized clinical trials support the use of coronary artery bypass grafting (CABG) for patients with unprotected left main coronary artery disease. Studies suggesting the equivalence of percutaneous coronary intervention (PCI) with CABG for this indication indirectly support the 2011 American College of Cardiology Foundation/American Heart Association Class IIa recommendati...
